Doctors have long been telling the Alzheimer’s Association how time-consuming and difficult it is to put together a comprehensive care for their patients in cognitive decline. So the Association decided to create a toolkit to help doctors help their patients.
Since early 2017, when the G0505 Medicare code went into effect, physicians can be reimbursed for taking the time to develop a care plan together with their dementia patients and their caregivers. Building a plan with your doctor is very important – it can empower patients when they are feeling most vulnerable. The new toolkit features a safety assessment guide, a caregiver profile checklist, an end-of-life checklist, best practices and an overview of the code to help doctors guide their patients along the path to care.
